Documentation of Disability
Individuals who request special services or accommodations must submit proof of their disability and supporting documentation from a physician, psychiatrist, or other appropriate healthcare provider, school or rehabilitation agency. Such documentation should include the following information:
- Diagnosis
- Nature of disability/limitations
- Expected duration of disability
- Statement of expected barriers/problems in a college environment
- Recommended accommodations and services
If documentation is given from a school district it should preferably be no older than three years, or the most recent that is available. Documentation should include a Psycho-Educational Evaluation and an Individual Educational Plan (IEP). In those cases where an IEP is submitted, please be aware that it must include testing modifications.
At the time the student applies for the CLEP test, disability documentation should be sent directly to the Office of Special Services. The office needs at least one month to review documentation.
Please write across the top of your documentation:
CLEP Individual (for CLEP testing only) or
ORP Individual (for Outside/Remote Proctoring)
