Required Acknowledgement of Meningitis Information
In accordance with New York State Public Health Law section 2167, all students enrolled
at Suffolk County Community College, both matriculated and non-matriculated, are required to acknowledge that they have received information about meningococcal disease and vaccination.
Students must indicate, on the Health History and Meningitis Acknowledgement form,
either that they have received the meningitis vaccine within the past 10 years or that they have decided not to obtain the meningitis vaccine. All students who have
received the vaccine must submit appropriate documentation of the vaccination.
Students who fail to submit the required acknowledgement will not be permitted to
register for classes and will be subject to withdrawal.
